All of the information you need is in the LICENSE file.  I do not have
anything to add, and I do not sign release forms.  The reason why I
didn't respond is because you should have read the manual first.

I'd like to be able to respond to individual requests, but my free
software packages (wwwstat, MOMspider, libwww-perl, and the Apache server)
are currently used at over half a million sites on the Internet, and
the only way I can avoid having to answer the same question hundreds
of times is to use an archived mailing list.
