"Unresolved" question

Our Unresolved hits this week were up to over 18%
I know that part of the problem is that our server is using Version 1.0,
and I have asked him to upgrade to ver.2.0 with the -dns option turned on.
However
1) Typically, what will be the percentage of unresolved hits even with
this advancement?
2) Are there a number of Government and Military entities that are
deliberately configured so that you don't know they have visited?
3) If not, why would a hit be Unresolved at all?
