Can your help me with Perl form...
I`m writing Perl program "The Qustionnaire"

1.some qustioin.....<br>
<input type="Checkbox" name="a_1"> yes <br>
<input type="Checkbox" name="b_2"> NO <br>
......
---------------------end htmlfile--------------------
I`m beginning learning languge Perl, I know can use a type="text" 
I do not know can I use type="Checkbox"
if you have some exampels ? Please give me.
