help with formmail

Can anyone help me with this problem? I am trying to use FormMail.pl
from Matt's Script Archive. Because my server requires cgi scripts to
have the cgi extention, I have renamed the script FormMail.pl to
FormMail.cgi. I changed the forms to reflect the new name and I still am
getting an error when I submit forms. The error reads:

Please contact the server administrator, and inform them of the time the
error occurred, and anything
you might have done that may have caused the error.

There was also some additional information available about the error:
[Mon Aug 4 12:52:26 1997] access to
/export/home/reppltd/site/cgi-bin/FormMail.cgi failed for
207.150.68.24, reason: Premature end of script headers 

I have used this script on other servers and so I know that it works.
What could be wrong?
