Need help with form cgi?

Hello:

I was given your email address and was told you are a "master" at cgi
scripts!  I have a specific need for a form mail script, there is
probably already one written, but I have not been able to find it.

When a user enters on a "form" 2 different email address, I need the
form submission to go to each of them and then both of the email
addresses forwarded to me (so I know who is using the form).
