OK, I see my post was confusing.

I've dumped the form to a file, for reasons you'll understand if you look at
it.  You won't be able to submit it but:
the url is http://www.netspace.org/~bperkins/tableform.html

GET -o html http://www.netspace.org/~bperkins/tableform.html
will obliterate the tables
while
GET http://www.netspace.org/~bperkins/tableform.html
shows the whole form.

you can try your favorite browser with tables and it will work fine
